Full-Time Quality Assurance Nurse Position Available
|

Supervisor:

|
|
Requirements:
|
- Must be a currently licensed LPN or RN in good
standing in the State of Utah
- Knowledge of Office Systems
- Minimum of two years’ experience in health care
field
- Strong Organizational skills
- Able to work flexible hours including weekends /
evenings
- Knowledge of Quality Assurance and Improvement
Principles
- Experience in compliance, auditing, and education
in home health and hospice required

Job Duties:
|
- Direct the QUAPI (Quality Assurance and Performance
Improvement) committee and report committee findings to the
Administrator.
- Direct clinical record reviews and report findings to the
Administrator at least quarterly.
- Develop the Agency Performance Improvement plan in
association with the Administrator.
- Assure compliance with all Federal, State, and local laws,
regulations, and statutes, as applicable, with regards to the Agency’s
provision of services.
- Follow and support the goals and policies and procedures of
Caregiver Support Network.

Certified Nurse’s Assistant (CNA)

Requirements:
|
- At least 18 years of age
- Evidence of current or ability to obtain
certification within 6 months of hire
- Maintain current CPR certification and evidence of
annual TB screening
- Able to read instructions of supervisor and write
observations in client record
- Clearly communicate verbally and in writing
- Must have own transportation to appointments
- Must have compassionate attitude
- Attend at least 12 hours of in-service per year

Job Duties:
|
The PCA may only perform tasks
as outlined in the Plan of Care for the client and as communicated by the
care manager. These tasks may include:
- Observe self-administration of medications by
reminding client to take medications and opening containers, but NOT touching
the medication itself.
- Assist the client, when needed, in personal
grooming, oral care, dressing and toileting.
- Light housekeeping, prepare meals and assist the
client with eating.
- When specifically approved by CSN, transport client
to appointments as scheduled.
- Take and record oral temperatures.
- Administer emergency CPR / first aide; contacting
physician or 911 if situation requires further care.
- Provide or arrange for social interaction and
recreational/diversional activities
appropriate for the patient’s cognitive disorder and as requested by
responsible family caregiver or supervisor.
- Document observations and services provided in the
individual patient record at the time of visit. Accurately complete
and timely submit the time log, as required. Sign, initial and date
all documents.
- Complete time log accurately, clearly, timely and
provide appropriate communication to Care Manager, and other support
specialists as needed, about client / family issues and response to
services provided.
- Demonstrate competency in all areas as outlined in
Performance Review.
- Communicate with Care Manager weekly on client’s
status.
- Comply with and support of company policies and
procedures.
- Other Duties as Assigned.
