Putting patient care first has made Caregiver Support Network first in the State of Utah.
Our carefully chosen specialty programs help our clients return home sooner, recover faster and remain as independent as possible. We continue to be the only licensed agency in Utah that specializes in dementia care.
The graph to the right illustrates the percentage of patients who stay at home after an episode of home health care ends — which exemplifies the true goal of home health care better than any other single measurement.
At Caregiver Support Network, our name really means something. The agency was founded in April 2000 to provide private duty services and therapeutic activity for people with Alzheimer’s disease and other dementia, at the same time focusing on supporting and educating their family caregivers.
Because patient care comes first at CSN, we conduct business differently from typical home health, hospice, and personal care agencies. For example, EVERY case is opened and followed by a registered nurse. We have no set, arbitrary numbers for nursing, therapy or aide visits - the patient’s level of care is predicated on his or her individual need. We conduct interdisciplinary team meetings weekly and offer in-depth, ongoing training on Alzheimer’s disease and dementia, one of our specialty programs. We have higher than average utilization of social work services to make sure our clients and families are connected to all available resources to reduce caregiver stress and insure the best possible long-term outcomes.
And we know if we take excellent care of our employees, they will provide excellent care to our clients. From Executives to Aides, we offer paid time-off and holiday pay, medical/dental benefits, a flexible spending plan, a 401K Plan and a profit-sharing program. According to our benefits broker, GBS, CSN was one of the few companies they work with that improves its benefits package each year, given escalating insurance costs.
This strategy has served us well. CSN is the proud recipient of HealthInsight’s Quality Award in 2006, 2007, and 2008 for excellent patient outcomes. CSN has to rank in the 90th percentile or above on publicly reported quality of care measures to receive this honor. HealthInsight is a private, non-profit organization that analyzes data collected by the Center for Medicare and Medicaid Services (CMS). Additionally, CSN qualified to be named in “The Home Care Elite,” the Top 500 home health agencies in the U.S., in the April 2007 edition of "Success in Home Care" magazine, the only Utah agency in this group. Also, in 2008, CSN qualified to be named Utah’s Best of State 2008 in the health care facilities category.
CSN is committed to providing innovative, comprehensive, individualized services based on client acuity and need through a holistic approach to care. We ascribe to the geriatric care management model which takes into account each client's physical, cognitive, emotional, social, financial, legal and environmental concerns. Fully utilizing this approach to patient care provides for the most cost-effective treatment while reaching optimal outcomes for the client and caregiver.
